A Customer may suspend his/her Customer Membership Agreement by submitting a completed Suspend Member Benefits form to Melaleuca bearing his/her original signature, printed name, address, Customer Number and reason for suspension. Forms received by Melaleuca on or before the 25th of the month will be effective the month received. Forms received by Melaleuca after the 25th of the month will be effective the following month.

The Suspend Member Benefits form must be submitted at Melaleuca Stores or Service Center.

Forms MUST be mailed to: Unit B-10-3A & 5, Level 10, Menara UOA Bangsar, No. 5, Jalan Bangsar Utama 1, 59000 Kuala Lumpur or may be handed in personally by the owner of the account at any of our Melaleuca stores.

1. Download Customer Membership Agreement form.
2. Fill out Section 1, making sure to select your Product Point commitment, and sign your name in Section 5.
3. On the top of the form, please hand write across the top "Changing Product Point Commitment".
4. Once complete, please submit via Document Upload Portal in Business Center or handed to the nearest store/service center.

The requirements for reactivation and reenrollment are found in the Statement of Policies #37. To reactivate your account,

1. Call 1-800-183-183 and speak to one of our friendly associates.
2. Visit any of our showrooms or service centers.
3. Via Document Upload Portal in Business Centres.
